That's a small barrell used to play the Cuá (two sticks) patterns that complement the many different "Seises" or "Sones" de Bomba.

Depending on the ensemble or region of Puerto Rico, some use a small barrell (like the photo you provided), other use bamboo pieces, and in the Bomba tradition from the South of Puerto Rico where Buleadores are bigger the Cuá is played on the side of one of the supporting drums.
